Trigeminal neuralgia is characterized by Lancinating pain in the 2nd and 3rd division of trigeminal nerve territory.

It is a paroxysmal attack of severe pain in trigeminal nerve, of short duration < 20 minutes, not associated with any other neuralgic sign.

Aetiology -  Most of the cases no aetiology cause is found.

• Disseminated sclerosis

• Facial hemiatrophy

Age- 50 - 70 years

Onset - Sudden, paroxysmal

• Trauma on face
• Exposure to cold or cold wind

• Mandibular division

Character of pain : (severe pain in intensity)

• Stabbing pain
• Lancinating pain
• Shooting pain

Spreading of pain

In all the three division of trigeminal nerve - over one half of face.

Duration - Few seconds to few minutes (Never more than 20 minutes)

• Carbamazepine Upto 1200mg daily.
• Dilantin sodium
• Baclofen 10-20mg 3-4 time / Day
• Clonazepam

Surgery

• Rhizotomy 
• Posterior tractectomy

If patient can not be controlled pain.

Mag phos is a very useful remedy for trigeminal neuralgia. There is radiating type of pain. Neuralgic pain relieved by warmth Mag phos is best suited to tired, languid exhausted subjects. It is a great anti spasmodic remedy. Vertigo on moving, falls forward on the closing of eyes, better by walking in open air.

Supraorbital pains, worse right side, relieve by warmth applied externally. Severe neuralgic pain worse behind the right ear, worse by going into cold air and washing face and neck with cold water. Laments all the time about pain.

General muscular weakness.

Doses- First to 12 potency. Sometimes the highest potencies are preferable.

Acts especially well, when given in hot water.

Hypericum

Excessive painfulness is the guiding symptoms for its use. Facial neuralgia and toothache of a pulling, tearing character with sadness. Pain in eyes and ear. Hypericum is best suited to the right sided facial pain.

Arnica

There is Neuralgias originated by the disturbances in the pneumogastric nerve. Best act in plethoric, dark haired people of rigid muscles, with a nervous, sanguine nature, but it acts feebly in debilitated people. Symptoms worse by least touch, motion, rest, damp cold and better by lying down, or lying with head low.

Pulsatilla   

It is a very useful remedy for trigeminal neuralgia. Best suited to right sided neuralgia with profuse lachrymation. Prosopalgia towards evening till midnight, chilly with pain.

Neuralgic pain, commencing in the right temporal region (migraine), with scalding lachrymation from the affected side. Otalgia worse at night.

Toothache relieved by holding cold water in mouth.

Frontal supraorbital pain. There is severe pain extends to face and teeth, vertigo, better by open air. The disposition and mental state are the chief guiding symptoms to the selection of pulsatilla. It is pre-eminently a female remedy, especially for a mild, gentle, yielding disposition. The patient seeks open air always feels better there, even though she is chilly.

Doses -Third to thirtieth attenuation.
