Classification

1.Primary/Idiopathic haemorrhoids

Cause

Standing position 

• The human being is partly responsible for piles because blood has to flow against gravity.

Anatomical Factor

• The veins passing through the submucous of the rectum,get constricted during the act of defaecation.

Familial or genetic

• Absence of valves or congenital weakness of vein wall.

Constipation

• Because of excessive straining.

          Thus gravity,straining and irregular bowel habits are important factor is developed of piles.

2. Secondary haemorrhoids

•  Haemorrhoids due to compression or superior rectal veins due to progesterone which relaxes smooth muscle in the wall of the veins,can causes haemorrhoids.

• Above the denate line, covered with mucous membranes.

External Haemorrhoids

• At anal verge, covered with skin.

Interno-external

•  Both varieties together.

Clinical Feature

•  Painless bleeding- Fresh bleeding occurs after defaecation-- Splash in the Pain. This causes chronic anaemia. Haemorrhoids which bleed are called Grade 1st haemorrhoids.
• The capillaries of the lamina propria are only protected by a single layer of epithelial cells,hence minor trauma precipitates bleeding.
• As the straining increase,the haemorrhoids partly prolapse outside. After defaecation,it returns back(Grade 2nd) or can be digitally replaced(Grade 3rd haemorrhoids).
• Permanently prolapse piles outside (grade 4th) haemorrhoids. Patient complaints of pain or discomfort.
• Most of the patient complain of constipation.
•  Discharge of mucuos and irritation of perianal skin pruritus is a common feature.

Complications of piles

• Anaemia - rarely massive bleeding can occur due to portal hypertension.
• Ulceration and secondary infection(may resolve)
• Thrombosis (black piles)
• Fibrosis(Firm pile)
• Pyaemia Gangrene (dry pile)

Nux Vomica
