About Acute Respiratory Distress Syndrome




This is the condition develop from various systemic and pulmonary disease.

The term 'Acute Lung Injury' not longer use.

It is serious disease characterized by damage to alveolar epithelium and pulmonary capillary endothelium.

Septicemia

Pyaemia

Shock

Injury

Venous embolism

Recurrent transfusion

Bypass surgery

Drugs-Narcotics, aspirin

Eclampsia


2. Pulmonary cause

Pulmonary infection- viral,bacterial, fungal

Toxic gas and smoke inhalation

O2 poisoning

Thromboembolism

Lung injury

Aspiration of gastric content

Drowning, hanging

Miliary tuberculosis

Inhalation-Ammonia, chlorine smoke, ozone.

Aspiration-vomitus, water (near drowning)

Hanging


Clinical features

  • Acute onset
  • Usually 12-48 hours
  • Laboured breathing
  • Cough
  • Sputum
  • Slight temperature
  • Feature of multiple organ failure present
  • Age-adult
  • Sex-both
  • Pulse-Tachycardia
  • Respiratory rate- Hurried
  • Chest- sucking of intercostal space


Investigation

X-ray chest- diffuse and patchy bilateral infiltrates

LFT- Pao2<50mmofHg

Pressure study- Normal pulmonary capillary wedge pressure


Treatment

  1. O2 Therapy
  2. Ventilator
  3. Blood transfusion
  4. Dopamine
  5. Nitric oxide inhalation
